Public Pool/Spa Permit Application or Public Pool/Spa Permit Update. Forms used when applying for a public pool/spa permit or when updating information on the original application; such as a change in ownership. The application contains facility operation information including Pool or Spa Location, Owner, Manager, and Emergency Contact information. A current copy is required to be kept on file with Environmental Health and will be checked at the time of inspection.
Water Quality Log. A form for use in complying with required record keeping for all Public Pools/Spas (California Code of Regulations, Title 22, Chapter 20, Article 3, section 65523). A current copy is required to be kept on file and will be checked at the time of inspection.
Anti-Entrapment Devices and Systems for Public Pool and Spa Compliance. This form must be filed by the owner of a public swimming pool within 30 days following the completion of construction or installation of anti-entrapment devices or systems in swimming pools.
